In this tutorial, I will explain you how to add a contact form on the contact page. There are two contact template ready in this theme. Health Center theme has support, contact form 7 plugin. You have to no need any code customization just simply paste your contact form short-code and see the magic. In this theme well-designed all form elements of contact form 7. 1. Install Contact Form 7 Plugin?
Firstly, You need to do install Contact Form 7 plugin. This plugin allow you to display your contact form on contact page. This plugin provides you a short-code. We need to enter short-code in page editor. How to install a Plugin? Click Here
2. How to get Contact Form Shortcode ?
Go to => WP-Menu => Contact => Contact Form or Add New
After install the plugin you can check WordPress menu by the name of CONTACT below on the settings menu. Click on the CONTACT menu then you will find your form short-code. Now copy this short-code and paste in page editor. Do the follow below steps. Refer attach screen shot.
3. Create a Contact Page?
Go to => WP-Menu => Pages => Add New
